Our updated pre-application is now here! You can now open up the word attachment from our website, fill out the forms and e-mail your pre-app back to us. Hopefully this will eliminate having to find a fax machine to fax back your application, skills checklist and consent and release form. The sooner you get these back to us, the quicker we can process your credentialing for your start date.
Included in the pre-application package is also our updated Skills Checklist for Speech Language Pathologists, Physical Therapists, and Occupational Therapists.
